La enfermedad de Kikuchi-Fujimoto (EKF) es una gran imitadora. Su presentación clínica se asemeja a entidades infecciosas, como también enfermedades reumatológicas, trastornos linfoproliferativos o lesiones metastásicas. Es una entidad benigna, poco frecuente, autolimitada y de etiología desconocida. Generalmente afecta los ganglios cervicales y su sintomatología es variada. La enfermedad de Kikuchi-Fujimoto (EKF) es una gran imitadora. Su presentación clínica se asemeja a entidades infecciosas, como también enfermedades reumatológicas, trastornos linfoproliferativos o lesiones metastásicas. Es una entidad benigna, poco frecuente, autolimitada y de etiología desconocida. Generalmente afecta los ganglios cervicales y su sintomatología es variada. A continuación, se presenta el caso de un hombre de 29 años, previamente sano, con linfoadenopatías generalizadas, pérdida de peso significativa no intencionada, fiebre alta intermitente, diaforesis nocturna y malestar general. La tomografía con emisión de positrones con deoxiglucosa marcada con flúor-18 (PET/CT 18F-FDG), reveló múltiples ganglios con actividad metabólica intensa sugestiva de malignidad, entidades infecciosas o enfermedades autoinmunes, las cuales se descartaron. El diagnóstico histopatológico confirmó una linfadenitis histiocítica necrotizante, compatible con la EKF, una enfermedad rara y subdiagnosticada, en la cual el reconocimiento temprano podría evitar intervenciones diagnósticas o terapéuticas innecesarias.

Kikuchi-Fujimoto disease (KFD) is a great mimic. Its clinical presentation resembles infectious entities, as well as reumatological diseases, lymphoproliferative disorders, or metastatic lesions. It is a benign, rare, self-limited entity of unknown etiology. It generally affects the cervical lymph nodes, and its symptoms are varied. The following is the case of a previously healthy 29-year-old man with generalized lymphadenopathy associated with significant unintended weight loss, intermittent high fever, nocturnal sweating, and general malaise. Fluorine-18-labeled deoxyglucose positron emission tomography (PET / CT 18F-FDG) revealed multiple nodes with intense metabolic activity suggestive of malignancy, infectious entities, or autoimmune diseases, which were ruled out. Histopathological diagnosis confirmed necrotizing histiocytic lymphadenitis, compatible with KFD, a rare and underdiagnosed disease, where early recognition could avoid unnecessary diagnostic or therapeutic interventions.
